Future Ready is a collaborative planning process that helps youth prepare for their future. It brings together key partners to create one clear transition plan, outlining actions, responsibilities, and goals for work, education, and community living as youth move from secondary school and children’s services to adulthood.

CSCN offers this service for youth over 14 years old in London/Middlesex, Huron, Perth, Elgin, or Oxford Counties who have complex needs and require more in-depth planning beyond what is provided by their school. A CSCN Service Coordinator will work with the family, the school, and others involved to develop a transition plan, and the family will receive a copy once it's complete.
